How should I list my address?

You should include your address under the heading of contact information. Keep entire contact info like address, email, phone number at one place. Mention that address where you are currently residing(where you would be available) . Mention your house no., city, district, state pin code

You dont need to list your address in your resume. It doesnt matter where you live as long as you are able to work in the city where the organization is located.
